The Missouri Department of Health and Senior Services respects your privacy and is committed to protecting your personal information. The Department and any provider under contract to establish and maintain the first-person consent organ and tissue donor registry (Donate Life Missouri Registry) and related sites are acting on behalf of the State of Missouri. Information submitted is used for official business. It is important that you understand our privacy position.
The Department’s Donate Life Missouri Registry system is part of the State of Missouri, a public governmental body. As such, it is subject to Chapter 610 of the Revised Missouri Statutes, commonly known as the Sunshine Law.
To ensure our system remains available to all visitors, our operating system tracks unauthorized attempts to upload or change information, or that can otherwise cause damage.
The State of Missouri uses network monitoring software to identify unauthorized web access to or tampering with our website(s). The Donate Life Missouri Registry system has implemented industry-accepted security safeguards to prevent unauthorized access and to protect the integrity of the system, interfaces, application software, system files, and personal data within the Data Center Services environment, disaster recovery locations, and any off-site archiving locations. If unauthorized access to or tampering with the site is detected, the information gathered will be furnished to the proper authorities. Registration authorizes electronic communication in such matters.
The information collected is information you provide voluntarily. You can access your record electronically at any time by visiting DonateLifeMissouri.org. Your donor information, by law, may be disclosed to other entities.
To complete your online transaction, you must provide the required information. Our online forms indicate which information is required and which is optional. You do not have to complete “optional” fields. If you prefer not to submit this information online, you can complete your transaction in person at your local DMV or send in a paper form. We are only responsible for adding or removing information provided directly through this site, by paper, or through a license office transaction.
The Department may gather aggregate statistics from registry data in a form that does not identify you to better understand registration trends.
Your email address is used to send a confirmation of your registry enrollment, removal or in the event your information needs to be clarified and/or verified. Your email address may also be used to notify you of any new information about organ and tissue donation, the Registry, and updates or issues directly related to your registration, including privacy policy updates.
A temporary cookie is stored on your computer’s hard drive during your visit to the Registry site. Cookies are used for authentication, tracking and to speed up your access to the site and the information you wish to see. Cookies do not contain any personal information. When you log off from the site, we automatically clear any temporary or session-based cookies. Most web browsers accept cookies. Check your browser menu for options to prevent, accept, or receive cookies. Please be aware that if you block cookies, some site functionality may be lost.
Financial Contributions
