I understand and agree to give my organs, eyes, and tissues to others after I die. This will be done by trained health care staff and in accordance with my record. Because I am saying yes, my choice is final. My family does not need to agree, and my choice will be respected.
I also understand and agree:
- Missouri law says I get to choose. Once I make my choice, it can't be changed unless I or someone allowed by law does so.
- If I change my mind about being in the Registry, I need to take the necessary steps to remove my name from the Registry.
- The information in the Registry is only shared after all attempts to save my life have not worked. My information may be given to other groups, like a recovery group, for the sole purpose of recognizing and acting upon my donor decision.
- My family cannot override my decision if I am 18 years of age or older.
- I agree and release the donor agency to tell my family about my decision when I pass away. The donor agency will also ask my family to help by sharing details about my life and health history.
- I can limit my donation to certain body parts and/or for specific reasons (transplantation, therapy, research, non-research) in my registry record.
- Under Missouri law, my gift can be examined to determine if my gift is healthy enough to use. Before using my gift, I understand that the exam includes a complete medical and social history.
- Results of the exams also helps find the best person or people to receive my gift.
- Tests may include blood and tissue samples. My test results might stop my organs, eyes, and tissues from being used for transplants.
- If the results of my blood test show any disease or condition that could affect others, I understand that these results will be reported as required by law.
- My gift will be used for transplant and therapy unless my record states otherwise. If my gift is not usable for transplant and therapy, my gift(s) may be used for research or education unless my record states otherwise.
- My donation will be used as allowed by Missouri law, no matter if the organization getting my donation is for-profit or non-profit.
- It is against the law for anyone to sell organs or tissue for any reason.
- My body may be moved to another facility for the recovery of my organs and tissues.
- My gift won't cost me, my family, or my estate anything after I die. The organ recovery agency handles costs for taking care of my donation. Any medical-related expenses before my death are the responsibility of my financial provider and estate.
- I release all parties, including the Missouri Department of Health and Senior Services and authorized donor agencies, from any responsibility related to my registration and donation.
- Access to Missouri’s Registry is restricted to authorized Department, contractor, and recovery agencies staff.
