Below are forms that you can use to document your decision to be, or not to be, an organ, eye and tissue donor; a family notification card; and a materials request form of materials you can request if you want to host a registry drive in your community. Google Translate was used to provide information in the Spanish language.   • Donor Card A printable card that you can fill out and carry with you that indicates you want to be an organ, eye and tissue donor. Inform your family of your decision. (Una tarjeta imprimible que puedes completar y llevar contigo en la que se indique que deseas ser donante de órganos, córneas y tejidos. Informa a tu familia sobre tu decisión.)
  • Enrollment Application (English) (02/25) A Registry enrollment application in the English language. Type in the form, print it, then sign and mail it to the address at the bottom of the form. A confirmation once the form has been processed. Inform your family of your decision.
